New Patient Appointments

To schedule a new patient visit, please call the appointment desk at 512.454.5821.

After scheduling the appointment, all new patients should fill in the following FOUR forms before their appointment:

• New Patient Information Form
• New Patient Questionaire
• Patient Medication List
• HIPPA Compliance Form

All new patients need to bring proof of insurance. Failure to do so may delay or prevent you from seeing the doctor.

HMO patients need to check on their benefits and have a referral before the clinic visit. Failure to do so may delay or prevent you from seeing the doctor.

Medical Insurance Plans:

We accept a wide variety of insurance plans. There is some variability and all doctors are not on all the plans. Most larger PPO Insurance Carriers are accepted. Some HMO Insurance Carriers are also accepted but it is the patient's responsibility to provide our office with a Doctor's Referral.
